Ordering by Fax

1. Create a hardcopy purchase order addressed to:

   Attn: GSA Administrator
2. Be sure to include the Great Lakes Electronic GSA Contract Number (GS-35F-0489J) on the purchase order. Don’t forget to include your name and phone number.
3. Fax the purchase order to 1-716-825-9623.
